Job Summary

Suburban Home Health is a leading provider of home health care services in central Indiana. We are seeking a skilled Home Health Aide to join our team and provide high-quality care to our patients.

Key Responsibilities
  • Provide personal care activities of daily living, including bathing, shampooing, oral hygiene, skin care/nail care, shaving, and dressing.
  • Communicate changes in patient status to the supervising Registered Nurse (RN) in a timely manner.
  • Report patient complaints to the RN.
  • Create and submit legible, accurate, and timely documentation.
  • Perform duties in compliance with agency policies and procedures and local, state, and federal regulations.
  • Demonstrate attention to detail for patient safety and safety of their home environment.
  • Report unsafe conditions in the home.
  • Practice effective infection control and adhere to infection control policies and procedures.
  • Practice safe lifting and transferring of patients.
  • Communicate clearly and positively with patients, families, team members, and others.
  • Display a caring, patient-first behavior.
  • Keep patients and/or family informed regarding visit time and schedule changes.
  • Attend in-service programs to meet compliance requirements.
  • Maintain patient confidentiality in compliance with HIPAA.
Requirements
  • Must be a Certified Nursing Assistant (CNA) and/or possess current Home Health Aide registration in the state of Indiana.
  • Must have a high school diploma or equivalent.
  • Must possess a valid driver's license and automobile insurance in the state of Indiana.
  • Must be able to provide own transportation to assignments.
  • Ability to work independently.
  • Ability to work in and support a team environment.
