- Review clinician productivity and report to the Director, Clinical Operations.
- Provide guidance to clinicians on patient care and answer questions and concerns.
- Lead weekly case conferences to review patient admissions, discharges, and re-certifications.
- Collaborate with the scheduler to review staffing needs and ensure proper patient care.
- Monitor on-call logs and ensure patient needs are addressed.
- Assist with new hire orientation and provide training to agency expectations.
- Monitor overtime, absenteeism, and associate ETO requests.
- Coordinate 24-hour on-call coverage and create on-call calendars.
- Track and review Tele-Health utilization and identify patients for the program.
- Manage and direct a team of clinicians to ensure safe and effective home care services.
- Meet with clinicians regularly to provide guidance and information.
- Provide direction to teams to ensure client needs are met and services are provided.
- Perform annual supervisory visits to determine staff competency and respond to educational and developmental needs.
- Assist clinical staff in establishing priorities, setting goals, and following up to ensure goals are met.
- Collaborate with the Director, Clinical Operations on process improvement, quality initiatives, and policy/procedure implementation.
- Educate clinicians on clinical services, policies, and procedures.
- Facilitate problem-solving sessions to enable Case Managers and other staff to resolve client and/or reimbursement issues.
- Serve as the clinical expert on home health guidelines, ACHC requirements, State Operations Manual, and Medicare regulations.
- Ensure accuracy, completeness, and timeliness of clinical documentation.
- Monitor open and closed charts regularly and participate in quarterly clinical record review.
- Assist in screening and interviewing of new Agency personnel and make recommendations to the Director, Clinical Operations and Human Resources.
- Complete corrective actions and deliver necessary performance discussions in collaboration with Human Resources.
- Provide direct patient care to adult and geriatric patients in the home to assist with staffing needs.
- Maintain accurate and up-to-date schedules for field staff and actively plan for productivity.
- Provide on-call backup for Case Managers, as needed.
- Perform other duties as assigned by the Director of Clinical Operations.
- Graduate of an accredited school of professional nursing.
- Minimum of 5 years' nursing experience in an acute care setting, preferably home health.
- Minimum of 3 years' home health experience.
- Must possess current license from the Indiana State Board of Nursing.
- Valid driver's license and automobile insurance with reliable transportation to perform job tasks required.
