POSITION SUMMARY

The Support Services Coordinator will provide comprehensive case management to individuals residing in our Emergency Housing/Shelter. This role involves assisting clients in achieving self-sufficiency through advocacy, goal setting, and connecting them with community resources and support services. The Coordinator will engage with clients on a personal level to establish objectives and monitor their progress.

KEY RESPONSIBILITIES

  • Collaborate with clients to create their initial personalized case plan.
  • Assist clients in completing necessary documentation for goal attainment, referrals, and additional paperwork as required.
  • Set specific goals in the following areas: 1) obtaining identification 2) securing employment 3) pursuing education or training 4) addressing addiction issues and mental health treatment 5) fostering spiritual growth through community involvement 6) achieving financial stability through budgeting and banking 7) complying with legal obligations 8) transitioning to sustainable housing.
  • Maintain accurate and objective case notes using the SOAP method to document client interactions throughout the week.
  • Keep a weekly schedule in the Client Management System for all case management appointments with clients.
  • Meet regularly with clients to assess their progress and ensure stability; prepare detailed and objective case notes reflecting these interactions.
  • Refer clients to appropriate agencies based on their needs and in line with their case plans.
  • Collaborate with volunteers to provide life skills training as client needs are identified.
  • Conduct regular checks to ensure clients' living spaces are maintained according to established guidelines.
  • Provide coverage for staff during breaks as needed.
  • Administer random drug screenings for clients when necessary.
  • Ensure the safety and security of the facility at all times.
  • Facilitate the discharge process for clients, ensuring accurate information is provided.
  • Meet weekly with the supervisor to update on client progress and challenges.
  • Utilize available resources to address the unique needs of clients.
  • Treat all clients with dignity and maintain appropriate professional boundaries.
  • Adhere to the core values of our organization in all workplace interactions.
  • Ensure clients comply with the established guidelines of the program.
  • Perform any additional tasks as assigned by the Assistant Manager or Program Director.
  • Share inspirational messages with clients when opportunities arise.
  • Engage in prayer with clients and staff as appropriate.

Qualifications:

Education: High School Diploma or equivalent required. Certification in Addiction Counseling is preferred.

Experience: At least one year of experience working with individuals facing homelessness or addiction challenges. Experience in ministerial counseling is preferred.

Confidentiality: Must handle highly confidential information with discretion.

Mental/Visual Demands: The role requires normal mental and visual capabilities.

Physical Requirements: Ability to stand for extended periods and lift up to 25 pounds. Must be able to bend or kneel as necessary.

Work Environment: Standard office setting.
