Psychiatric Mental Health Nurse Practitioner

Northwell Health is seeking a skilled Psychiatric Mental Health Nurse Practitioner to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and working collaboratively with our multidisciplinary team to ensure the best possible outcomes for our patients.
Key Responsibilities- Initial Patient Assessments: Conduct thorough initial patient histories and perform physical examinations to identify patient needs and develop effective care plans.
- Care Planning and Coordination: Initiate, document, and communicate patient care plans, ensuring seamless coordination with attending physicians, residents, and other care providers.
- Patient Rounds and Monitoring: Perform daily patient rounds to monitor patient progress, address concerns, and make adjustments to care plans as needed.
- Medication Management: Write orders for medications, laboratory work, and diagnostic tests, and interpret laboratory and test results to inform patient care decisions.
- Collaboration and Communication: Confer with healthcare team members to ensure optimal quality of patient care, and communicate effectively with patients and families to ensure understanding and compliance with care plans.
- Discharge Planning: Develop and implement discharge plans, including post-discharge care instructions and referrals as needed.
- Education and Certification: Master's Degree in a Nurse Practitioner Program and National Board Certification in specialty area required.
- Licensure: Current license to practice as a Nurse Practitioner in New York State, and current license to practice as a Registered Professional Nurse in New York State, required.
- Collaborative Practice Agreement: Has a collaborative practice agreement with designated protocols filed with NYS Department of Education OR collaborative relationship attestation as per the Nurse Practitioner Modernization Act 2015.
- Certifications: Current BLS certification required.
