Job Summary

We are seeking a highly skilled Medicaid Waiver Program Coordinator to join our team at West Central Nebraska Area Agency on Aging.

About the Job

The successful candidate will work as part of a team to provide support services to individuals and families in need, with a focus on coordinating Medicaid waiver programs. This is a challenging yet rewarding role that requires strong organizational skills, excellent communication abilities, and a passion for delivering high-quality care.

Responsibilities
  • Initial intake of Nebraska DHHS Medicaid Waiver clients and DHHS Medicaid eligible Nursing Home residents
  • Assessing client needs and developing comprehensive plans of care
  • Coordinating services to implement care plans, including level of care assessments and follow-up
  • Determining best pathways to provide and fund services for clients
  • Providing information on service options to enable informed decision-making
  • Using professional judgment to determine acceptable risk and intervene when necessary
  • Providing advocacy services to ensure access to resources and knowledge
  • Developing community resources in cooperation with resource developers
  • Assisting in service development where services are fragmented or nonexistent
Requirements
  • Bachelor's degree in human behavioral services fields such as human services, social work, psychology; OR be a registered nurse
  • At least two years of professional experience in long-term care, gerontology, rehabilitation, health/disability case management, children with special health care needs, or health/medical
  • Ability to travel to the Agency's seventeen county service areas
  • Proficiency with computer skills
  • Good verbal and written communication skills
  • Valid Nebraska driver's license
  • Professional liability insurance

Benefits

We offer a competitive salary range of $42,000 - $45,000 per year, plus benefits including 100% paid health insurance and flexible scheduling.
