About the Role

We are seeking a highly skilled and compassionate Intake Coordinator to join our team at CenterWell Home Health. As an Intake Coordinator, you will play a critical role in ensuring that patients receive the highest quality care and support.

Key Responsibilities
  • Initial Patient Contact: Respond to incoming calls from referral sources and potential patients, gathering information to identify patient needs and determine the company's ability to meet them.
  • Referral Management: Record the outcome of calls, make follow-up calls as necessary, and identify alternative community service resources when solutions are not available.
  • Patient Screening: Perform initial telephone screening to determine patients' appropriateness for home care services with referral sources, advising branch staff of patient acceptance and providing relevant information for a smooth transition to home care.
  • Payer Source Management: Contact referral sources to advise of referral status, provide information to the clinical team, and identify potential payer sources, verifying benefits and negotiating rates and authorizations.
  • Account Management: Access national or regional account information, communicate with branch and intake department staff as needed, and work with the Intake team and branch staff to identify potential solutions as problems arise with payer sources.
  • Customer Service: Monitor and track referral sources' satisfaction levels, communicating customer service issues to up-line Intake and branch staff as appropriate.
Requirements
  • Education: High School Diploma or equivalent
  • Experience: Minimum of three years healthcare delivery or related business experience
  • Skills: Strong knowledge of medical terminology, customer service focus, knowledge of insurance reimbursement process, effective data entry and word processing, problem-solving, human relations, and oral/written communications skills
