The Greater Nashville Regional Council (GNRC) is looking for a dedicated Social Worker to join its CHOICES Intake team within the Department of Aging and Disability Services. This department is committed to enhancing the quality of life for senior citizens in the region through effective planning and coordination of state and federal resources.
The CHOICES Intake team plays a crucial role in evaluating and assisting older adults who may qualify for the TennCare sponsored CHOICES program, which provides essential nursing facility services and home and community-based support for individuals aged 21 and older with physical disabilities, as well as seniors aged 65 and above.
Key Responsibilities:
- Inform and guide older adults regarding available programs and services that support long-term care, transportation, nutrition, and other essential needs.
- Conduct both telephone and in-person evaluations and assessments for clients throughout the Middle Tennessee area.
- Perform pre-admission evaluations and gather necessary financial and medical history from prospective clients.
- Stay updated on issues related to aging and disabilities, while collecting information on local resources and service providers.
- Assist in executing periodic customer satisfaction surveys to aid GNRC and TennCare in monitoring service quality.
- Document relevant information accurately within the designated systems.
- Undertake additional responsibilities as assigned by the CHOICES Intake Program Manager or the Director of Aging and Disability Services.
Qualifications: The ideal candidate should possess a bachelor’s degree in social work, hold an active social work license, or be a Licensed Practical Nurse (LPN). Strong verbal and written communication skills are essential. Experience in geriatrics and case management is highly preferred. Bilingual skills are an advantage. Foreign degrees and credentials will be evaluated.
Technical Skills Required: Proficiency in Microsoft Office and Adobe Acrobat is mandatory.
Travel Requirements: This role necessitates travel across GNRC's thirteen-county region. A valid driver's license and/or access to personal transportation is essential.
Remote Work Policy: This position is not remote. The selected candidate will be required to work onsite during core business days, with potential work-from-home flexibility at the Manager's discretion.
Compensation: Starting salary will be commensurate with experience and qualifications. GNRC offers a fully funded pension through the Tennessee Consolidated Retirement System and provides an optional 401k plan with a dollar-for-dollar match of up to 3% of gross salary. Further details regarding benefits can be discussed during the interview process.
GNRC serves as the regional council of governments for Nashville and its surrounding areas, functioning as one of nine regional economic development districts established by the Tennessee General Assembly. Additionally, GNRC operates as the designated Area Agency on Aging & Disability (AAAD) for the region, collaborating closely with the Tennessee Commission on Aging and Disability and TennCare.
