Job Summary:

Glss is seeking a skilled Healthcare Professional to join our team as a Registered Nurse in Consumer Services. This is a full-time position with a four-day workweek available after six months of employment.

About the Role:

This position is responsible for working closely with Case Management staff to conduct assessments in the home for Home Care and Group Adult Foster Care consumers. The successful candidate will develop personal care plans according to EOEA guidelines and determine consumer clinical eligibility for Medicaid-funded services in community and nursing home settings.

Key Responsibilities:

  • Conduct assessments in the home for Home Care and Group Adult Foster Care consumers
  • Develop personal care plans according to EOEA guidelines
  • Determine consumer clinical eligibility for Medicaid-funded services
  • Participate in interdisciplinary care planning

Requirements:

  • 3 years of clinical experience
  • 1 year of experience in community health preferred
  • Strong organizational and communication skills
  • Strong clinical assessment skills and strong computer skills
  • RN License Required, BSN Preferred
  • Bilingual preferred
