Job Overview

The **Senior Clinical Pharmacist** is responsible for exhibiting expertise in all aspects of pharmaceutical care, including the interpretation, transcription, and verification of medication orders.

Key Responsibilities

• Actively engage in daily multidisciplinary rounds and bedside discussions to enhance patient care.

• Conduct comprehensive drug utilization reviews to advocate for rational medication therapy.

• Lead and develop innovative clinical pharmacy services while mentoring and educating staff to ensure precise documentation of clinical activities.

• Review, interpret, and input treatment orders, ensuring the accuracy of dosage, treatment schedules, and routes of administration based on laboratory data and evidence-based practices.

• Collaborate with various hospital departments to support clinical pharmacy responsibilities and service lines.

• Design and implement clinical pharmacy services such as drug consultations, nutritional support, pharmacokinetics, anticoagulant monitoring, pain management, and renal dosing.

• Provide essential medication and practice-related education to patients, caregivers, and healthcare professionals.

• Work closely with medical and nursing staff to optimize pharmacotherapeutic aspects of patient care, including infusion and outpatient medication management.

• Offer guidance to staff and patients regarding medication uses, potential interactions, and side effects.

• Supervise the preparation of treatments, investigational medications, and manage drug returns while maintaining accurate records of patient prescriptions and inventory.

• Ensure pharmacy technician duties are executed thoroughly and safely, minimizing risks to patients.

• Analyze and evaluate various treatment options, making informed independent decisions.

• Assist in the training and mentoring of pharmacy students and residents.

Qualifications

Education: A Bachelor's degree in Pharmacy or a Doctor of Pharmacy from an ACPE-accredited institution is required.

Experience: A minimum of 1 year of clinical pharmacy experience in a hospital setting or at least 3 years of general hospital pharmacy experience is required, preferably in the relevant area of focus.

Completion of a hospital PGY1 or pharmacy general practice residency is preferred.

Certifications: A current state license to practice pharmacy is required, or licensure within a specified timeframe of hire. Basic Life Support certification is required upon hire, and Advanced Cardiac Life Support certification is required within a specified timeframe.
