Job Title: Public Health Nurse III

Craven County, NC is seeking a highly skilled Public Health Nurse III to join our team. As a key member of our healthcare team, you will be responsible for supervising and coordinating the overall care delivered to patients assigned to your care by the Hospice team.

Key Responsibilities:
  • Supervise and coordinate patient care, ensuring that patients receive high-quality, patient-centered care.
  • Develop and implement care plans that meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure that patients receive comprehensive care.
  • Provide education and training to patients and their families on disease management and self-care.
  • Participate in quality improvement initiatives to ensure that patient care is safe, effective, and efficient.
Requirements:
  • Graduation from a four-year college or university with a B.S. in Nursing.
  • Two years of Public Health nursing experience.
  • Current licensure to practice as a Registered Nurse in the state of North Carolina.
  • Valid driver's license.
