Nurse Care Coordinator

Care One Enterprise is seeking a skilled Registered Nurse to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and ensuring the well-being of our residents.
Key Responsibilities:- Provide Direct Patient Care: Assess, plan, and implement individualized care plans to meet the unique needs of each resident.
- Lead the Nursing Team: Supervise and coordinate the activities of Licensed Practical Nurses and Certified Nursing Assistants to ensure seamless care delivery.
- Collaborate with Interdisciplinary Team: Work closely with physicians, therapists, and other healthcare professionals to develop and implement comprehensive care plans.
- Ensure Regulatory Compliance: Maintain accurate records and ensure adherence to state and federal regulations, as well as facility policies and procedures.
- Communicate Effectively: Maintain open and transparent communication with residents, families, and healthcare team members to ensure timely and accurate information exchange.
- Education: Associate's Degree from an accredited school of nursing.
- Licensure: Current licensure as a Registered Nurse in the state of practice.
- Experience: At least one year of experience in a related nursing role.
