Work-Life Balance with Comprehensive Benefits The Element Care Geriatric Care Specialist is involved in the development, execution, and assessment of care strategies that align with the objectives, standards, and policies of the PACE model of care. This role requires expertise in delivering conventional social work services in a professional and dignified manner, aimed at assisting elderly individuals in maintaining safety and comfort in their residences and communities for as long as possible. This is a full-time role, Monday to Friday, 8 AM to 4 PM; no evening, weekend, or holiday shifts.

KEY RESPONSIBILITIES:

  • Engages in the Interdisciplinary Team's (IDT) initial evaluations, care planning, and ongoing reassessments of participant care.
  • Participates in IDT meetings; contributes actively by sharing relevant information, following up on assigned tasks, and aiding in the development of the participant's care plan.
  • Completes all designated assessments – including initial, semi-annual, annual, service requests, and significant events.
  • Evaluates the psychosocial requirements of participants and offers supportive counseling, collaborating with behavioral health professionals.
  • Facilitates admissions and discharges from hospitals, rehabilitation centers, and nursing homes as determined by the IDT, ensuring that necessary documentation is completed.
  • Assists in the transition of participants from community settings to long-term care, working closely with Medicaid specialists, skilled nursing facilities, and caregivers.
  • Organizes and leads family meetings as required.
  • Refers participants and their families to suitable community resources and acts as a liaison and advocate with community organizations.
  • Maintains professional, accurate, and timely documentation of social services in participants' medical records.
  • Conducts participant council meetings as assigned.
  • Collaborates with the Director of Social Work and Behavioral Health providers to ensure guardianship documentation is current and educates participants about health care proxies.
  • Works with the fiscal department to uphold participant insurance benefits and complete necessary fiscal documentation in medical records.
  • Reviews care plans with participants, guardians, and/or activated health care proxies as assigned.
  • Completes authorizations for home care and other approved services promptly and accurately.
  • Conducts home visits or skilled nursing facility assessments as needed.
  • Collaborates with the Palliative Care team; assists with end-of-life planning as necessary.
  • Communicates timely with relevant staff regarding disenrollment, transitions to long-term care, site transfers, and changes in participant or caregiver demographics.
  • Reports any allegations of abuse to the appropriate state agency; provides support and resources to participants as accepted; completes required documentation.
  • Must be able to pass a fit test; position requires the use of a mask where a tight seal is essential. Incumbent must not have facial hair that interferes with the respirator seal.
  • Performs additional duties as required.
  • Frequent local travel is expected.

QUALIFICATIONS:

  • Current Social Work licensure in the Commonwealth of Massachusetts at the Masters level (L.I.C.S.W. or L.C.S.W.) is required.
  • A minimum of one year of experience in Social Work providing traditional Clinical or Case Management services to a geriatric population.
  • Current CPR Certification or the ability to obtain certification.
  • Treat all participants with respect and professionalism.
  • Strong verbal, written, and listening skills with the ability to multitask in a fast-paced environment.
