We are seeking a skilled Registered Nurse to join our team at Communitas, Inc. as a Day Program RN. This is a full-time, 40-hour position, Monday through Friday from 8:00am to 4:00pm, with no on-call, nights, or weekends required.
Key Responsibilities- Provide healthcare services to individuals in our Day Program, assisting them in attaining and maintaining an optimum level of wellness throughout their lifespan.
- Communicate health care information to families and/or residential providers, ensuring seamless care coordination.
- Train newly hired employees on unique protocols monthly and as needed, promoting a culture of excellence and compliance.
- Administer medications and treatments as prescribed by the PCP, adhering to CARF, DDS, Medicaid, and DPH regulations.
- Ensure daily management of medical needs, providing physical support, and assisting with lifting and transferring members who have physical challenges as needed.
- Maintain medication books, appropriating sections of member medical files, and ensure program compliance with all regulations established by DDS, DPH, CARF, and/or Medicaid.
- Registered Nurse with current Massachusetts license. New Grads welcome to apply.
- Ability to communicate effectively, both verbally and in writing, with individuals, families, physicians, service providers, and case managers.
- A valid driver's license, acceptable driving record, and reliable transportation are required.
- Successful candidates will be required to complete a background check, including references, CORI, and fingerprinting.
- Competitive hourly wage of $41.00.
- Pay increase after 1 year of service, pending a performance evaluation.
- 4 weeks of vacation, 1 week of sick time, and 12 holidays.
