Responsibilities

The RN Case Manager will be responsible for:

  • Conducting patient visits and coordinating care with other healthcare professionals
  • Overseeing the frequency of visits for each patient episode
  • Consulting with physicians and office staff to ensure accurate and timely patient care
  • Collecting and analyzing patient data to inform care decisions
  • Attending regular case conferences and meetings to discuss patient progress and care plans

Requirements

To be successful in this role, the RN Case Manager must:

  1. Be currently licensed as a Registered Nurse (RN) in the state of employment
  2. Have a minimum of one year of clinical experience
  3. Demonstrate strong knowledge and skills in current nursing practice
  4. Possess a valid state driver's license and automobile liability insurance
