Role Responsibilities:
The RN Case Manager will work collaboratively with various healthcare professionals to oversee and coordinate patient care from admission through discharge. This role emphasizes efficient use of hospital resources, minimizing patient expenses, and advocating for patient needs.
Key Duties Include:
- Assessing discharge requirements and determining suitable post-discharge care levels, ensuring smooth transitions and preventing readmissions.
- Providing essential clinical information to external agencies and effectively communicating with the patient care team.
- Identifying and addressing barriers to patient care, while promoting process improvements within the department and broader healthcare community.
- Maintaining professional licensure and pursuing ongoing education. Advocating for patients' legal, social, and ethical rights while balancing resource management.
- Coordinating and facilitating educational opportunities for patients and their families.
Qualifications:
- Associate's degree from an accredited nursing program
- Current Ohio RN license
- 3-5 years of recent clinical experience in an acute care environment
