Job SummaryWe are seeking a highly motivated and compassionate Peer Specialist to join our Critical Time Intervention (CTI) team. As a Peer Specialist, you will play a pivotal role in community outreach, coordinating participants' needs, enhancing daily living skills, and advocating for them in cases of discrimination or healthcare inequities.
Key Responsibilities- Persistent and Assertive Outreach: Engage with participants using strength-based approaches, beginning during inpatient hospital admission or emergency department visit.
- Health and Social Needs Assessment: Continuously assess participants' health and social needs through CTI's conversational and observational assessments and formalized risk assessments tools.
- Discharge Planning: Participate in hospital discharge planning meetings to identify the best community resources for returning patients.
- Data Collection and Reporting: Collect and report data, as required, and work with the team leader, data analyst, and other CTI teams to use data to inform future care delivery.
- Appointment Navigation: Accompany participants to appointments, provide travel training, reengage them in community care, and address barriers to care.
- Comprehensive Psychosocial Assessments: Review documentation and conduct comprehensive psychosocial assessments to determine participants' medical, psychiatric, housing, and other social needs in the community.
- Progress Monitoring: Monitor, evaluate, and record participants' progress with respect to care plan goals.
- Team Collaboration: Attend and participate in team meetings and supervisory sessions.
- Education: High School Diploma or equivalent; higher education preferred.
- Certification: New York State Certified Peer Specialist preferred or required.
- Experience: Completion of an internship or comparable training in peer advocacy, experience in providing advocacy services to individuals with mental illness and/or homelessness.
- Skills: Knowledge of community and homeless resources, counseling principles and methods for mental illness and substance use disorders, treatment, rehabilitation, and community support programs.
