We are seeking a highly skilled and compassionate Primary Service Coordinator to join our team at Youth Villages. As a Primary Service Coordinator, you will play a critical role in providing care coordination services to youth and families, facilitating wraparound meetings, and collaborating with key stakeholders to develop and implement individualized service plans.

Key Responsibilities

  • Carry a caseload of 10-12 youth/families and provide care coordination services
  • Facilitate wraparound meetings to identify and address the unique needs of each youth and family
  • Collaborate with families and key players to develop and implement individualized service plans
  • Develop and update as needed the ICMP (Individualized Crisis Management Plan) to ensure the safety and well-being of youth and families
  • Provide crisis support 24/7 and triage crisis calls to determine the appropriate assistance
  • Complete care coordination activities as identified in wraparound meetings
  • Provide transportation to appointments and activities as needed
  • Monitor medication administration and ensure compliance with treatment plans

Requirements

  • Bachelor's degree in social services discipline with experience working with at-risk youth, adults, or families
  • Strong organizational skills and attention to detail
  • Excellent written, verbal, and oral communication skills
  • Ability to manage multiple priorities simultaneously
  • Basic computer knowledge and proficiency in software applications
  • Ability to maintain a flexible schedule and work non-traditional hours
