**Job Summary:**
We are seeking a compassionate and dedicated Day Habilitation Specialist to join our team at Mercy Home for Children Inc. As a key member of our support staff, you will play a vital role in helping individuals with developmental disabilities achieve their full potential.
**Key Responsibilities:**
* Work directly with consumers to develop and implement personalized Day Habilitation Plans, focusing on promoting independence and self-advocacy.
* Provide support with daily living skills, including personal hygiene, toileting, showering/bathing, dressing, grooming, and feeding, while respecting each individual's autonomy and dignity.
* Maintain accurate records of consumer progress, including valued outcomes, incident reports, and daily communications logs.
* Collaborate with supervisors and other team members to implement daily schedules, life skills activities, and community inclusion programs.
* Ensure the health and safety of consumers, while promoting their independence and self-advocacy.
**Requirements:**
* 1-2 years' experience working with OPWDD population, with potential for certification in Driver Safety, SCIP-R, CPR/First Aid, and AMAP.
* High School Diploma/GED and a valid, unrestricted N.Y.S. Driver's License.
