About the Role

LHC Group is seeking a skilled Physical Therapist to join our team in Orange, New Jersey.

This is a full-time opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals.

Responsibilities
  • Evaluate patient needs and develop effective treatment plans to improve functional status, mobility, and overall well-being.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and training to patients and their families on proper techniques and strategies for managing their conditions.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust treatment plans as needed.
Requirements
  • Current Physical Therapy licensure in the state of New Jersey.
  • Current CPR certification.
  • Access to a dependable vehicle and valid driver's license.
