About the Role

We are seeking a skilled Physical Therapist to join our team at VNA Health Group. As a Physical Therapist, you will be responsible for providing high-quality physical therapy care to patients in their homes.

Responsibilities
  • Assess and Evaluate Patients: Conduct thorough assessments and evaluations of patients to identify their physical therapy needs and develop personalized plans of care.
  • Develop and Implement Care Plans: Create and implement effective care plans that address the unique needs of each patient, in collaboration with physicians and other healthcare professionals.
  • Provide Skilled Therapy Services: Deliver high-quality physical therapy services to patients in their homes, including exercises, stretches, and other interventions to promote mobility, strength, and function.
  • Monitor Progress and Adjust Care Plans: Continuously monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
  • Communicate Effectively: Maintain open and effective communication with patients, families, and healthcare teams to ensure seamless care coordination and collaboration.
Requirements
  • Physical Therapy Licensure: Current licensure as a Physical Therapist in the state of New Jersey.
  • Experience: 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong Communication and Interpersonal Skills: Excellent communication and interpersonal skills to work effectively with patients, families, and healthcare teams.
  • Valid NJ Driver's License: A valid driver's license in the state of New Jersey.
