Job Title: Physical Therapist

We are seeking a skilled Physical Therapist to join our team in providing high-quality care to patients in their homes.

Job Summary:

The Physical Therapist will be responsible for assessing and evaluating patient care needs, developing and implementing treatment plans, and providing interventions to improve patient outcomes.

Responsibilities:
  • Evaluate patient's functional status and physical therapy needs
  • Assist physician in developing therapy plan of care
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning
  • Instruct and inform participating members of the healthcare team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals
Requirements:
  • Current Physical Therapy licensure in state of New Jersey
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
