VNA Health Group is seeking a skilled Physical Therapist to provide high-quality care to home health patients in South Orange, West Orange, and Newark.

About the Role
  • Assess and evaluate clients for physical therapy needs and develop personalized plans of action in collaboration with physicians.
  • Provide skilled physical therapy care to home health patients, promoting independence and improving quality of life.
  • Document all changes in client condition and update care plans accordingly.
  • Identify counseling needs and plan appropriate teaching strategies to support clients and their families.
  • Monitor quality and appropriateness of services provided, adhering to agency infection control and safety policies.
Requirements
  • Physical Therapy Licensure in NJ required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and effective communication and interpersonal skills.
  • Valid NJ driver's license.
