Job Summary

VNA Health Group is seeking a skilled Physical Therapist to provide high-quality care to patients in their homes in East Orange, New Jersey.

Responsibilities
  • Assess and evaluate patients for physical therapy needs and develop personalized plans of action in collaboration with physicians.
  • Provide skilled nursing care to patients in their homes, including physical therapy and education on self-care techniques.
  • Monitor patients' progress and update care plans as needed.
  • Identify counseling needs and plan appropriate teaching strategies to promote patient independence and well-being.
  • Adhere to agency infection control and safety policies, as well as organizational policies regarding absenteeism and appearance.
Requirements
  • Physical Therapy Licensure in NJ required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and effective communication and interpersonal skills.
  • Valid NJ driver's license.
