Job Summary:

Fairview Health Services is seeking a Visit Facilitator to support our clinic team in delivering exceptional patient care. As a Visit Facilitator, you will play a vital role in ensuring a positive patient experience, from preparing patients for appointments to supporting the care team during visits.

Key Responsibilities:

  • Prepare patients for appointments by collecting and detailing pertinent information, including medical records, medication reviews, and health questionnaires.
  • Assist in the rooming process by collecting vital signs and ensuring exam rooms are stocked with necessary equipment and supplies.
  • Support the patient connection to the provider during visits and act as a resource to ensure a positive patient-provider experience.
  • Complete visit wrap-up and provide after-visit summaries to patients, including recommendations for follow-up appointments.
  • Communicate efficiently with patients, providers, and care team members, demonstrating comfort with in-person and virtual technologies.

Requirements:

  • At least 1 year of customer service experience.
  • Proficient with technology, including computer hardware and programs, and smart devices such as phones or tablets.
  • Preferred qualifications include current nursing, medical, social work, psychology, or pharmacy students, or successful completion of NA, EMT, or HUC programs.
  • Experience with medical terminology, Epic Health Records, telephonic customer experience, and working in a healthcare setting is also desirable.

Desirable Skills:

  • Excellent customer service, public relations, and communication skills.
  • Ability to prioritize and work with a fluctuation in workload while working independently.
  • Ability to adapt to change and engage in ongoing process improvement.
  • Flexibility to work at other sites is encouraged.
