The Social Services Assistant plays a vital role in supporting the social service team by delivering administrative assistance throughout the patient journey from admission to discharge.

**Key Responsibilities**
  • Engage with patients/residents and their families to offer support and resource information.
  • Address concerns raised by patients/residents and their families.
  • Assist in the transfer of patients/residents within the facility to ensure a smooth transition and adjustment, documenting the process as needed.
  • Contribute to the healthcare decision-making process within the facility under the guidance of a qualified social worker or consultant.
