Key Responsibilities:
- Conduct thorough assessments to identify patient needs and develop personalized care plans.
- Collaborate with healthcare teams to ensure timely and efficient patient care progression.
- Coordinate and facilitate patient care transitions to ensure continuity and quality of care.
- Provide education and support to patients and families to promote self-management and independence.
- Identify and escalate barriers to patient care, ensuring timely resolution and optimal outcomes.
- Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services.
- Participate in multidisciplinary rounds to promote collaboration and effective communication.
Requirements:
- Certificate, Associate, or Bachelor's degree in nursing or social work.
- Experience in clinical or acute care setting preferred.
- LVN/LPN, RN, LBSW, LMSW, or LCSW licensure in the state of employment.
Work Schedule: PRN, Per Diem As Needed
