  • Identify and escalate barriers to patient care, ensuring timely resolution and optimal outcomes.
  • Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services.
  • Participate in multidisciplinary rounds to promote collaboration and effective communication.

Requirements:

  • Certificate, Associate, or Bachelor's degree in nursing or social work.
  • Experience in clinical or acute care setting preferred.
  • LVN/LPN, RN, LBSW, LMSW, or LCSW licensure in the state of employment.

Work Schedule: PRN, Per Diem As Needed
