Job Summary

Key Responsibilities

Collaborate with physicians, providers, and practice staff to identify patients who would benefit from care management services.
Develop and implement individualized care plans to address the unique needs of each patient.
Provide ongoing support and education to patients and their families to promote self-management and independence.
Work closely with patients and their care teams to coordinate care transitions and ensure seamless delivery of services.
Participate in regular team meetings and peer review activities to share best practices and improve care outcomes.

Requirements

To be successful in this role, you will need to have a bachelor's degree in a related field and experience working with patients with chronic conditions. You should also have strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.
