The Center for Urban Community Services is a leading provider of housing and social services, dedicated to empowering individuals to live successfully in the community.

Kelly Safe Haven is a 60-bed residence for chronically street homeless individuals who are living with mental illness and/or substance abuse.

The program offers assistance in obtaining and maintaining permanent housing, utilizing the Housing First/Harm Reduction Model to engage individuals who may not accept services.

The Care Coordinator is responsible for working with an assigned group of clients to help them achieve their housing and recovery goals, while also contributing to the team's and program's success.

Requirements:

Experience and Education Required:

CM 1: High school diploma and 2 years of experience

CM 2: Bachelor's degree or HS diploma or equivalent and 4 years of relevant work experience

CM 3: BSW and 1 year of relevant experience (excluding fieldwork) or Bachelor's degree and 2 years of relevant experience or HS diploma and 6 years of relevant experience

CM 4: 3 years as CM 3 (internal only)

CM 5: 3 years as CM 4 (internal only)

Demonstrated ability to provide services to a specialized population, computer literacy, and good written and verbal communication skills are required.

The ideal candidate will be able to work effectively as part of a multidisciplinary team and possess a strong commitment to the organization's mission and values.
