Responsibilities & Requirements:
The Licensed Practical Nurse (LPN) is accountable for:
- Exemplifying Kettering Health Network's core values of Integrity, Compassion, Balance, Excellence, Stewardship, and Teamwork through professional conduct.
- Delivering direct patient care in accordance with the State Nurse Practice Act and ensuring daily paperwork is submitted.
- Executing care plans established by the Nurse Manager and Clinical Coordinators.
- Providing precise and timely documentation that aligns with the care plan.
- Updating medication profiles as necessary.
- Assessing and educating patients and caregivers regarding disease processes, self-care strategies, health promotion, and disease prevention.
- Communicating any changes in patient conditions to the Family/Physician, with documentation in the clinical record, and implementing new physician orders.
- Conducting drug regimen reviews at each visit.
- Participating in care coordination by reporting identified needs for other disciplines to the Nurse Manager.
- Administering medications in accordance with established policies and procedures.
- Instructing patients and caregivers on medication administration, including infection control practices.
- Effectively maintaining and ordering necessary supplies, ensuring proper storage.
- Adhering to infection control protocols, demonstrating appropriate use of PPE, performing hand hygiene as per policy, utilizing correct bag techniques, and disposing of hazardous waste according to regulations.
- Complying with agency policies and procedures.
- Attending mandatory meetings, training sessions, in-services, and team meetings.
- Meeting productivity standards.
- Performing additional duties as assigned to ensure the efficient operation of the department.
Job Requirements:
- Valid State of Ohio Licensed Practical Nurse License
- Current CPR certification
