Practical Nursing Specialist

The Practical Nursing Specialist plays a vital role in delivering essential nursing care under the guidance of a Registered Nurse (RN) within the healthcare setting.
This role encompasses a variety of functions including the collection and communication of patient data, executing medical orders, and conducting therapeutic interventions tailored to the needs of patients across different age groups, all in alignment with the established protocols and standards of PAM Health.
Additional responsibilities may be assigned as necessary.
KEY RESPONSIBILITIES
Patient Care
Under the direct oversight of an RN, the Practical Nursing Specialist is entrusted with the care of designated patients during assigned shifts.
• Administers prescribed oral and intravenous medications.
• Ensures timely and accurate documentation in patient medical records.
• Regularly reviews and modifies care plans as needed.
• Measures and records vital signs.
• Provides care for ostomies.
• Observes patients for any changes in their medical status.
• Communicates relevant patient information to the physician as necessary.
• Changes wound dressings and collects various specimens, including blood and urine.
• Manages nasogastric tubes and provides nutritional support through these tubes.
• Inserts and maintains urinary catheters.
• Offers care for patients with tracheostomies and those on ventilators.
Reporting and Documentation
Collaborates in formulating the care plan, which includes multidisciplinary approaches, discharge planning, and educating patients and caregivers for post-discharge care.
• Evaluates the need for community resources to facilitate ongoing care.• Monitors patient progress and promptly reports any changes to the RN and physician.
• Identifies and accurately communicates adverse reactions related to medical conditions and treatments.
• Utilizes effective communication skills for reporting changes and documenting in medical records.
• Accurately records vital signs, intake and output, and other observations on designated forms.
• Assists the RN in acquiring physician orders to address the medical and nursing needs of patients.
QUALIFICATIONS
Education and Training:
Must possess a valid license as a Practical Nurse or Vocational Nurse in the relevant state. Current Basic Life Support (BLS) certification is mandatory.
Experience:
Preferred but not required: one year of experience in inpatient medical-surgical nursing.
Knowledge, Skills, and Abilities:
• Strong verbal communication skills to effectively interact with patients of all ages, their families, and staff.
• Proficient in reading and writing in English, including medical terminology.• Ability to document accurately and thoroughly in medical records.
• Cognitive skills to implement the nursing process for patients in a skilled nursing environment.
• Organizational skills to complete nursing tasks efficiently.
• Ability to inspire and guide non-licensed staff in patient care tasks.
• Capacity to build rapport and trust with patients and their families.
• Flexibility to adapt to changing circumstances and job requirements.
