Job Summary

Hospice Buffalo is seeking a skilled Registered Nurse to join our team as an On-Call Registered Nurse. This role requires a compassionate and dedicated individual to provide high-quality patient care in a home environment.

The ideal candidate will have a strong background in hospice and palliative care, with the ability to assess patient needs and develop effective care plans. Strong communication and interpersonal skills are essential for this role, as the On-Call Registered Nurse will work closely with patients, families, and healthcare teams to ensure seamless care delivery.

Key Responsibilities:

  • Provide comprehensive assessments to identify patient needs and develop individualized care plans
  • Manage patient pain and symptoms using interdisciplinary approaches
  • Organize and prioritize assignments to meet productivity standards and complete documentation requirements
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care delivery

Requirements:

  • Graduate of an accredited school of nursing with current RN licensure in the State of New York
  • Certification in Hospice and Palliative Care Nursing preferred
  • Valid Driver's License and vehicle required for use when on duty or on-call
