Integrated Support Services Coordinator

The Integrated Support Services Coordinator plays a pivotal role in enhancing the quality of life for individuals with intellectual disabilities. This position involves hands-on activities in personal care, health coordination, and active advocacy, ensuring the needs and rights of clients are fully supported in a community setting.
Key Responsibilities- Personal Care and Support: Assist clients with personal hygiene, dressing, and meal preparation, tailoring support to meet individual preferences and needs.
- Medication Management: Safely dispense medication and educate clients and families on proper medication management.
- Health Coordination: Coordinate healthcare appointments, manage comprehensive care plans, and ensure continuity of care with health providers.
- Community and Social Engagement: Facilitate client involvement in community activities, provide companionship, and support social interactions to enrich clients' social lives.
- Advocacy and Education: Advocate for clients' rights within various systems and educate them and their families on available resources and supports.
- Mobility Support: Assist with mobility and physical therapy exercises as required.
- Housekeeping and Errands: Conduct light housekeeping tasks and assist with errands to maintain a healthy living environment.
- Education: Bachelor's degree in Social Work, Psychology, Nursing, or related field preferred.
- Experience: At least 3 years in direct support, healthcare, or community services, with a focus on intellectual disabilities.
- Certifications: Required First Aid and CPR certifications; additional caregiving certifications are beneficial.
- Driving Requirements: Valid driver's license and auto insurance with uninsured motorist coverage.
- Interpersonal Skills: Excellent communication skills, compassionate, patient, and a strong advocate for client welfare.
