**Job Summary:**

The Internal Admissions Coordinator represents Avante at Mt. Dora, Inc. with area hospitals and other medical professionals, promoting a cohesive team approach within the facility. This role requires effective communication skills, both verbally and in writing.

Key Responsibilities:

  • Develop and maintain positive relationships with personnel, residents, visitors, and the general public.
  • Utilize knowledge of medical terminology to ensure accurate documentation and communication.
  • Proficiently operate software applications, including Outlook, Excel, and Word.
  • Work flexible hours and weekends as needed to meet the demands of the role.
  • Read and write in English with proficiency.
  • Assess all referrals and communicate with appropriate staff to ensure seamless coordination.
  • Communicate with case managers and online systems regarding pending referrals.
  • Maintain accurate documentation on all accepted residents, including 3008, PASRR, FL2, and UAI.
  • Provide the Business Office Manager with all financial documentation prior to resident admission.
  • Coordinate with family and physician to obtain necessary information for resident admission.
  • Communicate with external facilities and hospitals regarding bed availability.
