Qualifications:- Exceptional verbal and written communication skills.
- Ability to interact diplomatically with staff, residents, visitors, and the public.
- Familiarity with medical terminology.
- Understanding of Medicare, Medicaid, and various insurance plans.
- Proactive in exploring and integrating new methods into current admission practices.
- Proficient in Microsoft Outlook, Excel, and Word.
- Knowledge of Federal and State regulations pertaining to Skilled Nursing Facilities (SNFs) regarding admissions.
- Flexibility to work varied hours, including weekends as necessary.
- Proficient in reading and writing in English.
- Minimum age requirement of 18 years.
The responsibilities of the Internal Admissions Coordinator include, but are not limited to:
- Assessing all referrals and coordinating with relevant staff.
- Communicating with case managers and utilizing online systems for pending referrals.
- Maintaining comprehensive documentation for all accepted residents, including necessary forms.
- Providing the Business Office Manager with financial documentation prior to admission.
- Utilizing Integrated Referral Management (IRM) for tracking referrals and potential admissions.
- Initiating the Admission Notification form for distribution upon actual admission.
- Initiating the Discharge Notification form for distribution upon actual discharge.
- Verifying insurance coverage for all potential admissions.
- Obtaining necessary authorizations for applicable admissions.
- Completing the Admission Inquiry Tool.
- Finalizing admission paperwork within 48 hours of admission.
- Coordinating with families and physicians to gather required information for resident admission.
- Communicating with external facilities and hospitals regarding bed availability.
