Purpose:
The Internal Admissions Coordinator serves as a vital link between the facility and local hospitals, as well as other healthcare professionals. This role is integral to the marketing team and contributes significantly to the collaborative approach within the facility.
- Exceptional communication skills, both verbal and written.
- Ability to interact tactfully with staff, residents, visitors, and the public.
- Familiarity with medical terminology.
- Understanding of Medicare, Medicaid, and various insurance plans.
- Willingness to explore and implement innovative methods in admission practices.
- Proficient in software applications such as Outlook, Excel, and Word.
- Knowledge of Federal and State regulations pertaining to Skilled Nursing Facilities (SNFs) related to admissions.
- Flexibility to work varied hours, including weekends as necessary.
- Proficient in reading and writing in English.
- Minimum age requirement of 18 years.
Responsibilities encompass, but are not limited to:
- Evaluating all referrals and coordinating with relevant staff.
- Communicating with case managers and utilizing online systems for pending referrals.
- Maintaining comprehensive documentation for all accepted residents, including necessary forms.
- Providing the Business Office Manager with financial documentation prior to admission.
- Utilizing Integrated Referral Management (IRM) to track referrals and potential admissions.
- Initiating Admission Notification forms for distribution upon actual admission.
- Preparing Discharge Notification forms for distribution upon discharge.
- Verifying insurance details for all potential admissions.
- Securing authorization and levels for applicable admissions.
- Completing the Admission Inquiry Tool.
- Finalizing admission paperwork within 48 hours of admission.
- Coordinating with families and physicians to gather necessary admission information.
- Communicating with external facilities and hospitals regarding bed availability.
