Key Responsibilities:
- Administer infusion therapies, including intravenous medications, injections, and other necessary treatments to patients in their homes.
- Monitor patients for any adverse reactions and communicate changes in their condition to the healthcare team.
- Educate patients and their caregivers on infusion therapy and effective self-management techniques.
- Coordinate care with a multidisciplinary team, including physicians, pharmacists, and home health aides.
- Maintain precise and current documentation of patient care and treatment plans.
Qualifications:
- Valid Registered Nurse (RN) license in Oregon.
- A minimum of 1 year of nursing experience, with a preference for those with infusion therapy experience.
- Excellent organizational and communication skills.
- Ability to work autonomously as well as collaboratively within a team.
- Possession of a valid driver's license and reliable transportation.
