Job Description

At HDE HOME CARE LLC, we are seeking a skilled Registered Nurse (RN) to join our team as a Care Manager. This is an excellent opportunity for a dedicated professional to bring their expertise to our agency and make a meaningful impact in the lives of our clients.

The successful candidate will be responsible for maintaining accurate client care documentation, including assessments, service plans, and medication profiles. They will also provide leadership and guidance to our management and direct support professionals, assist with new employee orientation and training, and help develop policies and procedures.

Key Responsibilities:

  • Comply with relevant regulations and standards, including OAR chapter 333, division 536 and the Oregon State Board of Nursing OAR chapter 851, division 047.
  • Provide consultation and guidance to clients, caregivers, and staff as needed.
  • Assist with planning and implementing in-service and continuing education programs.
  • Monitor the quality of care provided and identify areas for improvement.
  • Administer client documentation and ensure accurate and timely completion of records.

Requirements:

  • Current Oregon State Board of Nursing, RN license.
  • Ability to read, write, and understand English.
  • Valid driver's license and reliable transportation.
  • Ability to work independently with minimal supervision.
