Your Role:

  • Implement, monitor, and assess housing and social service plans for families enrolled in various programs.
  • Collaborate with new residents to design individualized housing, mental, and medical treatment plans.
  • Advocate for families and individuals in securing entitlements on their behalf.
  • Assist clients in obtaining permanent housing and make referrals to special housing agencies.
  • Meet with clients to develop strategies to meet their housing needs.
  • Coordinate permanent housing searches and accompany families to view apartments.
  • Refer clients to additional services such as mental health and medical services.
  • Liaise with daycare and schools to monitor the admission and attendance of clients' children.
  • Follow up on child welfare cases, court cases, and parole/police matters.
  • Collaborate with Shift Supervisors and Resident Aides to support families effectively.
  • Participate in weekly Interdisciplinary Team and Administrative meetings.
  • Complete and enter client case management records in the DHS-CARES system.

Requirements:

  • Bachelor's Degree preferred, High School Diploma required.
  • Experience in social services, housing support, mental health services, or a related field preferred.
  • Strong organizational and communication skills.
  • The ability to work effectively in a collaborative team environment.
  • Proficiency in maintaining accurate and timely records.
  • Knowledge of housing resources and entitlement programs is a plus.
