Job Title: Housing Support Specialist

About the Role:

The Housing Support Specialist provides social service assistance to tenants in supportive independent housing, managed by Clinton Housing Development Company. This role is under the supervision of the Director of Social Services and the Supportive Housing Supervisor.

Key Responsibilities:

  • Provide case management, housing support, and coordination of services to help tenants meet goals and maintain housing.
  • Assess client needs, link clients to services, and plan a course of action towards stabilization and maintenance.
  • Eligibility screening for public benefits and facilitate collection of documentation to support applications.
  • Engage residents through outreach and provide information on entitlements.
  • Develop community linkages and refer residents to community providers to address their needs.
  • Advocate with social service agencies to secure entitlements.
  • Maintain client files, progress notes, and service plans using AWARDs.
  • Collaborate with staff and mental health collaterals as needed.
  • Participate in tenant selection committee and attend tenant and staff meetings.
  • Produce monthly reports and maintain relationships with Hudson Guild and Clinton Housing programs/employees.
