Job Description

We are seeking skilled Hospice Registered Nurses (RNs) to join our team of compassionate caregivers. As a Hospice RN, you will play a critical role in providing expert clinical care and support to patients and their families. Your primary responsibilities will include:

  • Providing direct patient care services to hospice patients and supportive care to the patient's family with a reasonable case load.
  • Acting as primary liaison between physician, patient/family, and hospice team.
  • Empowering patient's caregivers by educating them on care for their loved one.
  • Managing and collaborating with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to provide comfort, peace, and dignity.
  • Evaluating patient and family needs and assisting in the preparation and execution of the plan of care for each patient.
  • Maintaining all assigned patient clinical records.
  • Participating in company quality assessment and performance improvement programs.
