Job Description
We are seeking skilled Hospice Registered Nurses (RNs) to join our team of compassionate caregivers. As a Hospice RN, you will play a critical role in providing expert clinical care and support to patients and their families. Your primary responsibilities will include:
- Providing direct patient care services to hospice patients and supportive care to the patient's family with a reasonable case load.
- Acting as primary liaison between physician, patient/family, and hospice team.
- Empowering patient's caregivers by educating them on care for their loved one.
- Managing and collaborating with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to provide comfort, peace, and dignity.
- Evaluating patient and family needs and assisting in the preparation and execution of the plan of care for each patient.
- Maintaining all assigned patient clinical records.
- Participating in company quality assessment and performance improvement programs.
