Shift: 8am – 4:30 pm
As a vital member of the Emory Transplant Center, you will play a key role in our esteemed Kidney Transplant Program, recognized nationally for excellence. We are looking for a Transplant Coordinator - LPN to manage the health and care of a designated group of patients, including those awaiting transplants, post-transplant recipients, and living donors. Your responsibilities will encompass:
- Ordering and monitoring laboratory tests and imaging studies in accordance with established protocols.
- Communicating normal test results effectively.
- Notifying patients of abnormal results following provider guidelines.
- Managing communication and care coordination among healthcare providers and patients.
- Facilitating care management for specific patient populations.
- Ensuring seamless information flow between physicians and patients, while operating under the supervision of licensed healthcare providers.
- High School Diploma or GED is required.
- Completion of an accredited Licensed Practical Nursing Program.
- A minimum of 3 years of experience in patient care.
