Kidney Transplant Coordinator LPN

We are seeking a highly skilled and compassionate LPN Transplant Coordinator to join our team at Emory Healthcare's Kidney Transplant Program. As a key member of our transplant team, you will play a vital role in ensuring the smooth transition of patients through the transplant process.
Responsibilities- Coordinate patient care and communication with healthcare providers, patients, and families.
- Order and follow up on lab results and imaging studies as per protocol.
- Triage and manage patient messages and concerns in a timely and efficient manner.
- Facilitate care management of defined patient populations.
- Coordinate information between physicians and patients, ensuring seamless communication.
- Graduate of an accredited Licensed Practical Nursing Program.
- 3 years of patient care experience.
- Excellent communication and organizational skills.
