Job Title: IDD Care Management Supervisor

We are seeking a highly skilled and experienced IDD Care Management Supervisor to join our team at RHA Health Services. As a key member of our organization, you will be responsible for leading and directing the operations of Care Management services, ensuring sustained high quality of care and services to individuals with intellectual, developmental disabilities, mental health, and substance use challenges.

Key Responsibilities:
  • Lead and supervise a regional team of care managers and care manager extenders, providing guidance and support to ensure effective delivery of care management services.
  • Collaborate cross-functionally with other RHA departments to ensure success of Care Management implementation, including clinical, operations, finance, IT, nursing, QA/training, etc.
  • Develop and implement workflows, policies, and procedures that promote whole person care, ensuring compliance with company, state, and federal policies.
  • Monitor and support staff in delivery of care management per the identified needs of beneficiaries, maintaining a positive and professional image in the work setting and community.
  • Ensure timely recruitment to fill vacancies and ensure continuity of services, managing staff training and development, and ensuring timely completion of performance evaluations.
  • Implement the strategic use of data to support the effective delivery of Care Management services, utilizing metrics and reports to ensure work is allocated timely and appropriately and meets regulatory compliance requirements, individual needs, and performance standards.
  • Maintain a safe and healthy environment, ensuring implementation of all organization health and safety policies and procedures.
  • Work with the Executive Director to execute, monitor, and analyze the program budgets in collaboration with financial services, ensuring operations are in accordance with the approved budget and productivity targets of the team are met.
  • Establish and maintain frequent and regular communication with employees, colleagues, payors, and peer network, fostering open communication, feedback, and helping keep the team focused and progressing toward organization goals.
  • Create and manage an ethical culture in conjunction with the RHA Code of Conduct and Ethics and Compliance Program, emphasizing confidentiality and no retaliation policies.
Requirements:
  • A license, provisional license, certificate, registration, or permit issued by the governing board regulating a human service profession, including LCSW, LMFT, LCAS, LCMHC, LPA, or RN license issued by the North Carolina Board of Nursing.
  • Three years of experience providing care management, case management, or care coordination to the population being served.
