Job Title: IDD Care Management Supervisor

We are seeking a highly skilled and experienced IDD Care Management Supervisor to join our team at RHA Health Services. As a key member of our leadership team, you will be responsible for managing and directing the operations of Care Management services, ensuring sustained high-quality care and services to individuals with intellectual and developmental disabilities, mental health, and substance use challenges.

Key Responsibilities:
  • Lead and direct the administrative, clinical, financial, and employee relations functions of Care Management services.
  • Implement and ensure compliance with company, state, and federal policies.
  • Directly supervise care managers and care manager extenders.
  • Develop and implement workflows, policies, and procedures to promote whole-person care.
  • Provide leadership to employees, promoting harmony and clearly stating organization objectives and strategies.
  • Maintain a positive and professional image in the work setting and community.
  • Monitor and support staff in delivering care management services per the identified needs of beneficiaries.
  • Ensure timely recruitment to fill vacancies and ensure continuity of services.
  • Manage staff training and development, ensuring policy and training development and implementation.
  • Collaborate cross-functionally with other RHA departments to ensure success of Care Management implementation.
Requirements:
  • Licensure or certification in a human service profession, such as LCSW, LMFT, LCAS, LCMHC, LPA, or RN.
  • Three years of experience providing care management, case management, or care coordination to the population being served.
  • Supervisory experience in a care management or related field.
  • Strong leadership and communication skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
