Patient Outreach Coordinator

Under the general direction of the Clinic Operations Manager, the Patient Outreach Specialist will provide patients/individuals with the education needed about our services, assist with the enrollment process, support new membership projects, retention assistance, and develop relationships with strategic community partners.
Key Responsibilities:- Enter new patient demographics and update established patient information, as necessary, in the Electronic Health Record (EHR) to maintain accurate patient medical records.
- Obtain new and established patient insurance information, including eligibility and authorization, as required.
- Assist patients in applying and/or aligning their healthcare coverage plans, providing necessary follow-up and data collection.
- Assist patients in navigating the enrollment process to secure needed services, including working on new additions and loss of patients in various networks.
- Outreach and schedule patient appointments as necessary to ensure membership retention and growth for the group, including phone calls, email outreach, and letter preparation.
- Outreach and educate patients regarding how and where to access healthcare services, including new services and providers.
- Assist in conducting assessments of retention efforts and identifying areas for improvement.
- Demonstrate flexibility with work schedule and assignments, including floating to other medical offices.
- Promptly answer and respond to all calls in a professional manner, acting as a liaison between patients, referring and treating physicians, staff, hospitals, and others involved in patient care.
- Identify patients' important needs, answer questions, and assist with informed patient decisions prior to and/or post office visits.
- Provide patients with practical resources and offer emotional support and understanding throughout the patient care coordination process.
- Outreach to patient membership that has overutilized hospital services.
- Maintain quality customer satisfaction.
- Monitor patient requests and outgoing messages for various patient experience initiatives, such as Follow My Health.
- Assist with front office duties as necessary to support patient experience and operations, including clinical setting deliverables.
- Prepare welcome materials for new patients and current patients.
- Participate in planning discussions and meetings, assisting in developing tactical plans to support outreach and patient membership goals.
- Prepare and submit weekly productivity reports.
- Minimum Education: High School Diploma required; College Courses and/or Associate Degree in Healthcare Administration preferred.
- Minimum Experience: 3-5 years of healthcare experience in customer service, administration, and/or patient care coordination required.
