LPN Care Specialist

Adoration Home Health and Hospice is seeking a skilled LPN Care Specialist to provide high-quality patient care in the comfort of their own homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities
* Provide one-on-one patient care in a home setting, including administering medications, monitoring vital signs, and documenting patient information.
* Collaborate with our interdisciplinary team to develop and implement patient care plans that meet the unique needs of each individual.
* Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
* Participate in ongoing education and training to stay up-to-date on the latest best practices and technologies in home health care.
Requirements
* Current LPN license in good standing
* Previous experience in home health, hospice, or hospital settings preferred
* Strong problem-solving and critical thinking skills
* Ability to thrive in a fast-paced environment
* Reliable transportation and valid driver's license
