Job Summary

The Advanced Practice Provider at Prevea Health is a highly skilled healthcare professional responsible for delivering high-quality patient care services to individuals, families, and groups. This role requires a strong foundation in nursing or medicine, with a focus on promoting health, preventing disease, and providing compassionate care.

Key Responsibilities
  • Assess, evaluate, and manage patients with various health conditions, including minor, acute, chronic, and episodic illnesses and injuries.
  • Collaborate with patients to review their health background, determine their needs, and advise them on future health plans and illness prevention.
  • Provide case management services to assist clients and families with arranging for or referring patients to needed services.
  • Conduct telephone calls to patients requiring follow-up care or provide test results, and offer health counseling and education to patients and their families.
  • Order diagnostic and laboratory tests, prescribe pharmacological agents, and provide non-pharmacological treatment modalities as needed.
  • Maintain and review patient records, charts, and other pertinent information, documenting all details in the electronic medical record.
  • Collaborate with physicians and other providers to develop and implement patient care plans to ensure the best possible care and outcomes for patients.
Requirements
  • Graduate of a master's level nurse practitioner program or graduate from an accredited physician assistant program.
  • 1-3 years of work experience in a clinical setting within a healthcare organization.
  • Knowledge of human anatomy, physiology, and pharmacology, as well as understanding of symptoms, treatment options, and preventive healthcare measures.
  • Excellent organizational, communication, and interpersonal skills, with the ability to form strong relationships with patients and peers.
  • Ability to prioritize work, meet changing demands, and handle confidential information.
  • Licenses and certifications as a Nurse Practitioner or Physician Assistant, including CPR/Basic Life Support certification.
