Position Summary

We are in search of a dedicated Client Support Coordinator to join our committed team in providing exceptional services. In this position, you will be key in facilitating the support and growth of individuals with special needs. Your responsibilities will include coordinating personalized plans and advocating for the rights of clients, ensuring that compassionate and effective services are delivered.

Key Responsibilities
  • Conducts mutual discovery processes to understand client needs.
  • Oversees the creation of personalized plans and organizes client-related meetings.
  • Monitors client progress and updates the personalized service plan accordingly.
  • Offers information on alternative options that influence program participation.
  • Coordinates the formulation of health care plans as necessary.
  • Acts as a strong advocate for clients.
  • Generates referrals for suitable services.
  • Provides crisis intervention when required.
  • Observes clients in community settings as needed.
  • Maintains client records, ensuring compliance with organizational and funding standards.
  • Establishes and nurtures relationships with EXCEED programs, funding agencies, and client support teams.
  • Participates in assigned meetings and contributes to discussions.
  • Develops and submits timely reports on client outcomes and demographic information to the Program Manager.
  • Assists in organizing community outings and volunteer opportunities for clients.
  • Facilitates training and participates in activities as necessary.
  • Performs other related duties as assigned.
Qualifications & Requirements
  • Must be at least 21 years old.
  • Possess a high school diploma or equivalent (GED).
  • Preferably have completed a minimum of two (2) years of college education.
  • Hold a valid Driver's License with a satisfactory driving record.
  • Maintain state minimum auto insurance coverage.
  • Pass pre-employment drug screening.
  • Obtain CPR and First Aid Certification within 30 days of employment.
  • Secure Live Scan clearance from relevant authorities.
  • Pass Criminal Background Clearance as mandated by health and welfare regulations.
  • Exhibit strong English grammar and writing skills for documentation and report preparation.
  • Demonstrate the ability to develop objectives, assess physical, academic, and psychological needs, and build rapport with stakeholders.
Physical and Mental Abilities
  • Possess sufficient dexterity for filing, typing, and leading meetings.
  • Exhibit adequate mobility for community access.
  • Demonstrate capable hearing for effective verbal communication.
  • Engage in various physical activities including walking, standing, and lifting.
  • Able to lift up to 50 pounds and perform tasks requiring bending, climbing, and balancing.
  • Maintain good vision and hearing for effective communication.
Schedule & Compensation:

Monday through Friday, working a 40-hour week (8-hour days).

Salary: $19.95-$20.95 per hour.

Reports to: Program Manager

Employment Status: Non-exempt.
