We are seeking a highly skilled and experienced professional to join our team as the Director of MDS Operations at Ardent Health and Rehabilitation Center LLC. This is a key leadership role that requires strong clinical expertise, excellent communication skills, and the ability to coordinate and manage the development and implementation of resident assessments.
Key Responsibilities- Resident Assessment and Care Planning
- Conduct and coordinate the development and completion of preliminary and comprehensive resident assessments (MDS) in accordance with current rules, regulations, and guidelines.
- Ensure that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, mental, and psychosocial needs.
- Collaborate with healthcare professionals to develop and update comprehensive resident assessments and care plans.
- Leadership and Coordination
- Supervise and coordinate the work of MDS staff to ensure timely and accurate completion of assessments and care plans.
- Develop and implement policies and procedures to ensure compliance with regulatory requirements.
- Collaborate with other departments to ensure seamless integration of MDS services with other healthcare services.
- Quality Improvement
- Monitor and evaluate the quality of resident assessments and care plans to ensure they meet regulatory requirements and best practices.
- Identify areas for improvement and develop strategies to address them.
- 2-4 years' experience providing direct care to multiple patients/residents, preferably in a Skilled Nursing environment.
- Previous experience in MDS is required, with supervisory experience preferred.
- Nursing degree from an accredited college or graduation from an approved LPN program.
- Current, unencumbered license to practice as an LPN in Colorado.
- Current valid CPR certification.
