Job Summary

As a PRN Home Health LPN with HCA Virginia Healthcare at Home, you will have the opportunity to provide quality, skilled nursing care to patients in the comfort of their own homes. Our team is dedicated to delivering exceptional patient care and support, and we are seeking a compassionate and skilled LPN to join our team.

Responsibilities
  • Administer quality, skilled nursing care to patients in the home, working in accordance with physician orders and the Plan of Care under the direction and supervision of a Registered Nurse and/or Director, Professional Services.
  • Provide patient care and identify the physical, psychosocial, and environmental needs of patients, verifying the Plan of Care prior to each visit and providing care according to the physician's orders, assessment data, established standards and guidelines.
  • Perform an ongoing assessment during each visit and document data in patient medical records, communicating significant findings, problems, or changes in the patient's condition to the supervising RN and/or Director of Professional Services and the physician.
  • Assess patient/caregiver's knowledge of disease process, treatment regimen, and/or medication administration, providing basic and specialized teaching as ordered in the Plan of Care and based on the patient's needs.
Requirements
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date.
  • Licensed Practical/Vocational Nurse (LPN/LVN) certification.
  • Drivers License.
  • Vocational School Graduate.
